Fix a bug when flash hook is pressed on the second outgoing call during ring-back state

Scheduling callback function for the timer of flash hook on the 2nd RINGBACK (R5 unattended
transfer) was wrongly set to create a 3-way conference.
Implement a new call back function that switches back to the held call in this case.
