Removed the timeout ubus requests to asterisk or dectmngr to prevent locks

In some abnormal cases, e.g. asterisk or dectmngr restart, no ubus requests will be
sent to dectmngr any longer. This issue has been fixed in this commit.

Miscellaneous improvements in log messages, logic simplification, and etc. are also
included.
